What Information Do We Collect About You?

Your name, date of birth, address and source of income are examples of personal information.  Your family history, the records of your appointments and what services we provide to you are examples of other information that may be in your record.

Information may be collected from you as well as from other sources who may have knowledge of you and/or the circumstance for which we are involved with you.  In most circumstances your consent for the collection of information about you from other sources will be sought except as follows:

  • under circumstances as permitted by law (such as when we are receiving a report pursuant to the duty to report in the CYFSA); or,
  • when information is reasonably necessary to provide child welfare services or to assess, reduce or eliminate a risk of harm to a child, youth or group of children and/or youth and it is not possible to collect this information directly from you that can be reasonably relied on as accurate and complete, or obtained in a timely manner.

The CASLM will limit the collection of information to what is required to protect children and youth and/or to look after children and youth in our care as defined within our legal mandate and related regulations.
